Our insurance coordinators deal with many different insurance companies. Some companies offer many different dental and medical plans. Some companies combine dental and medical coverage. This insurance alphabet soup changes policies and guidelines weekly. At times, it is almost impossible to accurately estimate our patients' insurance co-payment. Many insurance companies will not give out fees until after the treatment is completed. Dealing with these companies can be difficult and time consuming. As a courtesy, we ask that you keep us informed of any change to your insurance. It is important that all information about you and your insurance be current.

If You Have Insurance:

  1. You will need to pay the consultation fee at the time of the consultation. We will submit to your insurance company for reimbursement.

  2. We will need to verify coverage and eligibility before treatment begins. We can do this by submitting a predetermination of benefits for you. The information needed by us to verify this data is to be provided on your registration forms.

  3. You will need to pay us your portion of the fee quoted, either prior to or at the time of surgery, based on your insurance coverage.

  4. When we have received payment from your Insurance Company, we will credit your account with the payment we received from them. If this payment, together with the payment from you results in our receiving more than the fee we quoted to you, we will send you a refund check for whatever overpayment was received.

If You Do Not Have Insurance:

  1. We ask for payment as services are rendered.
  2. We will discuss individual payment arrangments at your consulation appointment.

If You Are an Emergency Patient:

You will have discussed your problem with the person answering your phone call, and will have been given an estimated fee, within a range. This fee will be due and payable in full (100%) at the time of the surgical procedure.

We know questions can arise on insurance matters, and these should be discussed with our Office Manager. We will be happy to help you receive maximum benefits, however, the agreement of the Insurance Company to pay for surgical care costs, is a contract between you and the Insurance Company.

A Final Word...No Smoking Please!:

Exposure to tobacco is hazardous to your health and we ask that you refrain from smoking while in this office.
