Your initial appointment will consist of a consultation. We will review your medical and dental history as well as collect clinical information necessary to diagnose the treatment you need. This clinical information may include a complete periodontal charting, x-rays and photographs. Please assist us by bringing the following information with you . A complete list of any medications you are taking including non-perscription medications such as aspirin and vitamins. Your referral slip and any x-rays that are applicable. Note: It is better to pick up the x-rays from your dentist and hand carry them with you to assure they are here for your appointment. If you have dental insurance, please bring the necessary completed forms or insurance cards. This will allow us to process any necessary claims. Important: All patients under the age of 18 years must be accompanied by a parent or guardian at the consultation visit as well as at any appointments needed for treatment. Additional Information: If you have a heart murmur, artificial heart valves and / or joints or a history of rheumatic fever, you need to take an antibiotic premedication for this consultation appointment.
